Insurance Coverage and Claims Procedures

Florida Atlantic University's athletic department insurance is a Secondary Insurance policy. The student-athlete's personal insurance is considered the primary insurance. All bills incurred from an athletic injury treated through the FAU medical system will be processed through the athletes/parents primary insurance. After the athletes/parents insurance has been processed, FAU's secondary insurance policy will pay the unpaid balances. It's very important for the athletes and their parents to send copies of medical bills and Explanation of Benefits (EOB) from their insurance companies to the Sports Medicine Department so bills can be processed in a speedy manner.

The athlete is responsible for providing complete and accurate information to the Sports Medicine Department. Florida Atlantic University will not be responsible for any medical charges accumulated due to inaccurate or false information. The athlete will be responsible for these medical charges. The athlete is also responsible for reporting any changes in their medical insurance coverage or insurance information to the Florida Atlantic University athletic training staff immediately. Failure to do so will result in the student-athlete and his/her family being held responsible for any medical bills received.

If a student-athlete is under both parents insurance policy, please send copies of both insurance cards (front and back). Also, completely fill out both parents portion on the medical insurance information form.

PLEASE INCLUDE A COPY OF YOUR INSURANCE CARD (FRONT AND BACK) WITH THE MEDICAL INFORMATION YOU ARE RETURNING TO FLORIDA ATLANTIC UNIVERSITY.

If you are not covered on your parents policy and/or do not have your own insurance, you must purchase insurance to be allowed to participate in any Florida Atlantic University athletic function. This insurance must cover the entire academic and competitive school year. Feel free to contact our staff if you or the parents of the athlete need recommendations of insurance polices to purchase.

International students: If you do not have an international policy, acceptable by the State of Florida Statues and regulations through the Office of International Studies, you will be required to purchase insurance. If you provide a written statement from your insurance company, in English, with the following requirements, we will honor your insurance. It must state that you have coverage for any injuries incurred during varsity sports practice or competition, it must explain the claim process, and must list a representative in the United States to contact for claims.

Reporting Injuries: Any injury incurred during organized practice or competition while representing Florida Atlantic University will be cared for by the Florida Atlantic University Athletic Medical Staff (athletic trainers, physical therapists, physicians, and other medical staff). Injuries should be reported immediately to the athletic training staff. Injuries not reported to the Florida Atlantic University athletic training staff within 72 hours after the injury, are not the responsibility of the Florida Atlantic University athletic department. Second Opinions Student-athletes desiring medical attention other than that provided by the Florida Atlantic University medical staff for injuries occurring as described above might do so at their own expense. For the athletic department insurance to be responsible for a second opinion, prior approval must be authorized by Florida Atlantic University team physician or medical staff. Students, who seek medical care other than through the Florida Atlantic University medical staff, must be cleared by a Florida Atlantic University team physician prior to returning to active participation.

Pre-existing/Non-Athletic Injuries & Illnesses: All injuries and medical abnormalities pre-existing prior to Florida Atlantic University enrollment and competition, are the responsibility of the student-athlete and his/her family. Disqualifications from participation in Florida Atlantic University intercollegiate athletics because of said abnormalities is the prerogative of the Florida Atlantic University team physician.

Injuries or medical conditions not related to organized practice or competition are the responsibility of the student-athlete. Florida Atlantic University athletic department will not provide coverage or pay bills incurred for expenses related to non-athletic illnesses, injuries, or conditions. Student-athletes are required to have a current and thorough medical insurance policy for non-athletic related injuries/illnesses. (Example of what athletic insurance will not cover: appendectomy, tonsillectomy, ulcers, pregnancy, ovarian cysts, hereditary conditions, hospitalization for medical illnesses, pre-existing injuries, non-athletic injuries/illnesses, automobile accidents, and routine dental care).

Primary Care Physician Recommendation: Student-athletes will spend almost 10 months of the year away from home at college, so it would be in the best interest of their health and well being to have accessible local medical services. We strongly recommend that all student athletes temporarily change their Primary Care Physician to a local doctor within the network of their insurance. This will make the process of referring an athlete to a specialist when sick or injured, rapid and uncomplicated. If assistance is needed in finding a doctor, please feel to call our staff.
