Dear Athletes and Parents:

Welcome to the Florida Atlantic University athletics program. I hope your summer break is going well. The Florida Atlantic University athletic training staff is here to assist you in any way we can while you are a student-athlete at Florida Atlantic University.

Enclosed you will find some important forms that must be read and completed before you will be allowed to participate in any Florida Atlantic University athletic activity. Please take time to read and complete these forms and return them in the enclosed addressed postage paid envelope that you will receive in the mail from the training staff. These forms must be received no later than August 6th, 2007. You will not be allowed to participate in any athletic activity until all forms are signed, dated and completed!

The following forms must be completed in full and returned:

1. Follow-up Athletic Medical History Form (Return Athletes Only)
2. Primary Athletic Medical History Form (Freshmen and Transfers)
3. Supplemental History Form
4. Medical Consent Form
5. Assumption of Risk Form
6. Parent/Student Insurance Information Form including a copy of your insurance card (front and back); prescription card and dental card.
7. Prescription Release Form
8. A Copy of Immunizations (freshmen and transfers)

ALL STUDENT-ATHLETES MUST ATTEND THE PRE-PARTICIPATION PHYSICALS AT FLORIDA ATLANTIC UNIVERSITY BEFORE THEY ARE ALLOWED TO PARTICIPATE IN ANY ATHLETIC ACTIVITY. Please check the date and time assigned for your team.

All student-athletes are required to have their own health insurance. Florida Atlantic University carries athletic insurance as a secondary policy. This is explained in the Insurance Coverage and Claims Procedure section. Please send a copy of your insurance card (front and back) with your completed forms. If you have a prescription card and dental insurance please send a copy proof with your completed forms.

Upon the initial physical exam, the Team Physician may require an athlete to receive an echocardiogram to rule out the presence of a heart abnormality. If it happens that these tests come back abnormal, the student athlete will be required to have a follow-up evaluation with their primary care physician and or cardiologist. These follow-ups are not covered by our school's secondary insurance policy, because these abnormalities are pre-existing conditions. Until the student athlete is cleared in writing to participate in activities, they will not be able to compete in any university sports related activities. We strongly recommend that all freshmen and transfers have a Cardiac Evaluation with their Primary Care Physician and or Cardiologist prior to arriving at Florida Atlantic University.

All freshmen and transfers must send us a copy of their most recent Immunization records, in addition to the copy required by Student Health Services.

Please print in black ink or type all forms. All forms must be completed in their entirety and signed. Do not leave any blanks. These forms must be returned no later than August 6th for Fall sports and August 24th for all other sports. If you have any questions, please contact me 561-297-2044.
